A developmental questionnaire asks you about skills most kids have at that age. You can learn if your child is on track or if she needs a little extra practice to be ready for school.

Ready for a snapshot of your child’s development? WithinReach offers the Ages and Stages Questionnaire (ASQ) to families in Washington State. Complete it online when it is convenient for you. It only takes 10-15 minutes to complete. One of our child development specialists will call you to talk about the results. They can suggest activities to support your child's growth at home in the community.

What if my child seems to be doing just fine?
The ASQ is a great way to make sure that your infant, toddler or preschooler is healthy and ready to learn and grow. You can give your child the best start possible by learning about his or her development and doing activities that will support them as they grow. It’s also a fun way to spend quality time with your child and learn what they can do!
What if I have concerns about my child’s development?
The ASQ does not diagnose a delay or disability, and it is not meant to replace a health check-up with your child’s doctor. But it may show if there is a need for further evaluation. If you and your child need extra support, we will connect you to specialists who can help put your child on a path to success.
What do I do next?
When you click the start button above, you’ll go to the online ASQ. Based on your child’s age, you will be asked about your child’s ability to complete certain activities, such as “Can your child stand on one foot?” When you’ve tried each of the items with your child, submit your answers online. Our expert staff will score the ASQ and contact you with results within one week. We will also give you suggestions for games and activities to do with your child to practice emerging skills. We will send another screen every 3-6 months, so you can continue to learn about and support your child’s growth.

Professionals Who Screen
Many kinds of providers use a screening with families. Many tools are designed to be completed by parents and scored by a provider who has been trained to interpret the results. Your pediatrician might use a screening tool during your well child visits. Childcare centers might use one at the beginning of the school year. Ask the providers in your child’s life if they check child development using a quality tool.
